Me, I'd accept it, and go on like nothing is out of the ordinary. Several of my exes are on my facebook, some I get along with, and some I can't stand(Especially one that went Christian on me and tries to push it on me whenever we speak).

Don't reject or accept it. I typically just let it sit there if I don't want to friend people. That way there's no way they can send me another one.
You should go on the offensive, stalk his facebookz and post creeper things until he is forced to uncontact you and leave you alone forever.

No, bad, awful idea. If he wants to try to get back together with you, which is a possibility regardless of how bad the breakup was, that will only encourage him.
If you want nothing to do with the fuck just deny his request. Don't be a pussy about it and keep it sitting there untouched forever like some suggest, either accept it and deal with the guy or deny it outright.
