Medieval II: Total War and if you want fantasy check our some of the mods.
Mount and Blade Warband is borderline free much of the time and has a lot of good mods which add some fantasy while already involving open world medieval combat and a learning curve you need a sherpa for.

I actually have MTW2 already, and bought Rome most recently. I've heard a lot about Mount and Blade, I may have to finally give that shot once I see what borderline free is.

Majesty 1 & 2 are pretty fun. The twist is you don't really play the part of the heroes, but rather you the play the part of the town hiring and creating missions for them. I've sucked quite a bit of hours into that game simply because of this different mechanic.
