When Nero heard Urahara say that... he hit him into the wall. He was NOT insane! He was a Hollow... sort of. He used to be a Hollow and the definition for insanity in regards to Hollows was debatable. They all had problems mentally but thats what happenes when you have a million or so voices and wills inside you fighting for control... at least when your below Vasto Lorde.

He admitted he had... quirks but he wouldn't go on killing sprees... unless they were other Hollows... or someone really, really, really pissed him off... like in 1944. He must have killed at least a hundred humans and thousands of Hollows in just a day or so. But he hadn't enjoyed it!

Yes he did. They had deserved it... the humans anyway. He was being merciful when he put down the Hollows. But nevermind that, he was NOT insane.
