Worst GM ever imo would do all of the following: 
Places too many restrictions on the players
Forces them to follow his script
Is a complete and total asshole
Egotistical and makes the RP about showing off their character (RP focused only around them over the other players) 
Makes up things out of the blue to keep players in line, this includes unnecessary rules like "All players must bow to the princess other than my character, who everyone must be afraid of" 
Instead of guiding the RP to flow a certain way, they force it...
kicking out the wolves and not listening to any second opinions
Thinking they are always right and that everyone else is wrong
ect...

The list is VERY long but it's how many wrongs vs rights a GM has that matters... 

Though this does give me an idea... 101 ways to be a horrid GM.
