I would normally use Photoshop or GIMP, but both of them like to crash whenever I go above 2 layers, so I use Paintdotnet with a few plugins.

As for creating the land itself, I used a bit of Indonesia/Philippines/Iactuallydontknowwhattheareaiscalled and messed with the levels until it was all one colour. If you want a randomly seeded land, you can use the generate clouds tool. I'll send you some examples when I get back on a computer.
