Well, it'll either be the result of you doing a dangerous action that you know has a chance of killing you and you roll poorly, or you actually do something very dumb (whether you know it or not, perhaps due to a failing to gather information or figure out a trick due to issues with either Quickness, Intelligence, or Perception...)... or maybe as an intentional act of sacrifice... or maybe it's the law brought down on you from the gods if you do something they really don't want you to do.
