1. Yes it would have been operated in America but where, that's unspecified. It can't have been too far away from the academy since Avery had no money. 

2. It is assumed so. It would have been around 60 people. Avery also assumes her parents are dead as her mother was extremely ill and her father most likely spent all the money on alcohol.
