This isn't the whole list, but it's some of it. I ripped it straight from part of a character sheet I've got stored in a few places, then I edited in how relates to your character sheet by using brackets. I've deliberately left out some of the more complex categories, and I'll only bring them up if you request me to.

Character Sheet

Full Name: (Pretty much the real name section you've already got listed. Sometimes I list this as real name instead of full name, but having it as full name encourages people to include the surname and even the middle names if they exist.)
Also Known As: (This is for titles, aliases, nicknames, et cetera.)
Birthdate: (Depending on how and when the role-play it set, I consider this more important than simply listing age. Basically, so long as you have a date set for your role-play - which I always do - it's easier to make an assessment of someone's age based on the date instead of having age listed, especially since ages can change depending on how long the role-play goes for.)
Place of Origin: (This is - if I'm not mistaken - what you listed as the character's nationality.)
Ethnicity: (Where we're born and our ethnicity can be different. For example, I'm Australian but my ethnicity is - more or less - Greek-Scottish. I say 'more or less' due to there being a vague possibility I've got Turkish blood in me as well, but for all intents and purposes the only people who could confirm this are dead.)
Gender: (You have this listed as the character's sex. I prefer gender, simply because it prevents people from putting 'yes please' as a smartass answer, but then no one's going to do that seriously anyway. Also, sometimes I include sexual orientation in a character sheet, so having gender instead makes the categories more diverse.)
Age: (You've got this.)
Height: (I consider this category pretty essential, especially since those who just give a picture might not mention it.)
Weight: (Less important than height, especially in a setting where people have superhuman strength, but still useful to have. Female characters more frequently have this listed as unknown than male characters, but can still be a useful category in a few regards.)
Laterality: (Right-handed, left-handed, or ambidextrous. Can be useful.)
Blood Type: (This is only useful in certain circumstances, but it can sometimes be good to have for those situations.)
Occupation: (Presumably all your characters are music idols, so this would be more for secondary jobs if they have one.)

Feel free to use as many or as little of these categories as you like, and that goes for anyone else who reads this.

I once read that Blood Type in Japamese media is an indication to a characters personality.

I think I incorprate a fee of these things in there. I like the thought of including Laterality. It's a nice detail. Thanks for this Sho~

You're definitely right about Japanese culture believing that blood type impacts the personality, and - while it's not the only resource - this gives an example. If this story is set in Japan, it might be a good idea to include that category simply because people around your characters are going to somewhat judge your characters based on it.

The category is also useful in role-plays that might include a hospital at some point, since blood transfusions or donations could occur. One other instance where it's useful is when there's vampires in a setting but these vampires actually work more in accordance with reality -- in real life, if you mix blood that doesn't match your own it can kill you, so fiction never seems to account for why a vampire won't get sick or die if they drink the blood of someone whose blood doesn't match their own.

Use as much as you'd like to, and no problem. Also, some of the more complex categories I left out include things like the character's marital status, residential status, how many languages the character can speak and what languages they are, and more complex classifications I won't go into unless someone asks me about them. I also left out anything that was unique to my project's setting.

I also left out the 'Astrological Sign' category, since I wasn't sure if you'd want to include it and the last time I sent this character sheet to someone it was for a role-play in a fictional setting that (presumably) wouldn't have used our astrological signs.

It's always affected me, I've seen one horror movie, wasn't scary at all. Rather it was more funny than anything to me, I own a number of horror games but I cannot play them, physically I can, mentally I cannot. Been life this my whole life really.

Und then I have writers block so I cannot reply to anything nor can I apply to this roleplay I'd like to.

I noticed you mention a few things in the visual novel thread that's going on recently. You might end up finding some of the things that get brought up in the Discord chat - for my project, which is not up yet but will be as soon as possible - interesting.

I don't even like Discord. I'm mostly setting up the discussion channel on it due to several people I know not having - or liking - Skype. Also, one of my friends was, to the best of my understanding, part of the development process of Discord and has a place set up already that could be converted into my channel.

This friend's basically one of the main people who's encouraging me to turn my project into a series of kinetic novels instead of just novels. He knows people who'd want to get involved in voice acting if it happens, et cetera.
