It's a bit of a long story, since it's a continuation of the Madoka Magica anime. Basically, her name is Nagisa Momoe. She's a dead girl who jumped into a twisted world created by one of the creatures know as "Witches" to help her friends. In life, she was a former "Witch" herself, called Charlotte, and looks like that as a "Witch" with an obsession with cheese. She took that disguise to hide herself, then revealed her true, human form that way.
