Fun fact. I was accused of a conspiracy during WOTW because a guy entered with a character that was marginally weaker than most. And lost. First round.

Second fact. @melonhead entered with a weaker character as well and made it to the final round. Won I think.

No, you are free to use whatever format you wish. I would look to characters I've accepted for ideas on what kind of information I'm looking for.

This will depend on the behavior of the spell. Most magic will require some form of preparation, not necessarily announcement. You are free to use magic however you feel is appropriate.

While Tuujaimaa is still working on his character, his would be a good example to follow in terms of a spellcasting type character.
