I'm not really sure, I have to say Joseph cares about others more than himself. Seeing a patient die before his eyes is definitely going to hit him hard, especially since he was helpless (to his knowledge) to stop it. Therefore I'd have to say being helpless to help those he cares about would be devastating to him.
