Your form looks alright to me, but you are missing the appearance section. Aside from that, you need only change the HP and MP values to not be d6s and just x/x. Twiglett is very cute!

Instead of writing down a whole appearance I added a picture to the stats, to give a clearer depiction, hopefully that is okay too.
