The next achievable form will rarely ever be used, only in cases of nobles.
The last achievable form will be during the final fight between the True Forsaken One and Shinji. After that, you may never see it again.

Allow me to elaborate a bit more on this whole "crystalline seams" thing. You probably all know how, in any crystal, there is a weak point, where hitting that point hard enough will cause the crystal to shatter? This is kind of like that. It doesn't really change the body, per se. Instead, it creates a force distribution effect and hardens the area just a little bit, so that when the area is struck just so, the force is distributed to tear up the area in a crystalline formation. Does that make sense?

There's a trigger. It's not supposed to be a cheap attack; there's a very specific guideline. To be fair, if you don't want to get a really bad hit, becoming a sitting duck in battle isn't a good plan.

Also, I cannot find a picture of shinji's new form, so I'll explain it:
Half of him is shrouded in darkness. His very being on that half emmits a dangerous and dark energy. If one could see through the darkness, one would see what looks like uqluiorra but withpitch black skin and sharp fangs.
The other half shines so brightly that it is hard to see.
It's like a person made up of entirely of light, but its not light, its good energy(not sure how else to put it)
The dark half represents the mistakes he has made in his life, the people he killed, the horrible things he has committed. The light half represents his true intentions, the goodness that truly resides in his heart.
