People are usually compliant due to fear, not respect. I've considered this extensively in the past and I think a blacklist would be more effective than a whitelist.

1. Don't make it "public" per se (it would be "flaming" or whatever, I'm sure), but make it available to anyone who requests access, via email/newsletter
2. Anyone can contribute to the list, as long as they provide evidence of the players' wrongdoing, including thread URLs and screenshots

5. Would probably have to set an arbitrary inactivity threshold;, like, if you go without posting for one full month, 30 days on average, then you are "officially" considered inactive. Just an example.
6. Is it possible for someone to get himself off the list if he wants to? Proving remorse?

3. It's crowdsourced, so it's not a bunch of power-hungry megalomaniacs arbitrating who's on the list and who isn't for personal, political reasons; it's by roleplayers, for roleplayers, especially GMs who can check the list before reading people's apps and accepting them into the RP.

4. Maybe different offense tiers?

Tier one: people who express interest in an Intchk but never show up for the IC/OOC thread

Tier two: people who sign up for an RP, posting in the OOC/Char tabs, but rarely or never actually go IC

etc.
