Place a 'character portrait' picture into the 'Image' BBCode, and make sure it's appropriate for this role-play. I don't care if you've drawn it yourself or gotten it from somewhere, but keep it fitting. The reason I've listed the 'Anime/Manga' tag for this role-play is that I'm envisioning it as an anime (not inherently inspired by anything specific), and subsequently any character pictures used are expected to follow that sort of style of art for the purpose of maintaining proper aesthetic. If you've got a problem with anime or its style, then this role-play isn't for you. Character portraits are obligatory for a few reasons, so make sure you have one here. Also, make sure it's accurate -- if your character is meant to have a different hair colour (for example) to the portrait or is meant to have freckles (another example) then use an image program to correct it.
Name: This should be obvious, but include the whole name of the character (given name, surname, et cetera). If you don't include a middle name (or more than one middle name for characters with more, like myself in real life), then I'll be assuming they don't have one.
Birthdate: Month Day, Year A.D.
Place of Origin: City, State, Country
Keep in mind that your character isn't obligated to have been living in this location when they received the letter to come to the event, and where you were when that happened will obviously be included in your biography.
Ethnicity: Where a person's born and their race can be entirely different, so that's why this is being included. If you've got more than a single ethnicity (Eurasian, for example, or something like Greek-Scottish) then list it here instead of trying to simplify it.
Gender: Male, Female
Pick one of the two, though if you really want to go Hermaphrodite I won't stop you.
Age: Pick something reasonable for the character portrait, biography, and the fact that this is a human.

Height is obligatory (and should be realistic), but you're allowed to list 'Refuses to reveal' or something similar for the weight. If you do list weight, make sure you're realistic about it (there's websites to help with this). If you don't, just keep in mind that sometimes situations come up where a character's weight is relevant, meaning you might get asked in the OOC. Here's some conversion help:

Occupation: Self-explanatory, but include your character's job here, including anything they do part-time. If the character's unemployed, that should go here, as should a job they aspire to if they're currently struggling to achieve it (Aspiring Writer, et cetera). Multiple jobs can be listed here, so long as it's something the character could realistically be multitasking. If they're a student then it's included here.
Marital Status: Single, Engaged, Married, Widowed, et cetera
I'd considered 'Relationship Status' instead (so you can list 'Dating' if you've got a boyfriend/girlfriend, et cetera), but this feels more professional to me. Anyway, basically just make sure the information is accurate. If the character's currently going through a divorce or separation then it might be worth mentioning too.
Appearance: A picture's acceptable on its own for this section, but isn't obligatory like the portrait at the top of the character sheet. If you do use a picture here, I expect it to be a full body shot so that the entire appearance of the character can be understood, and - unless you specify details in written form - I'll be assuming this is exactly what the character looks like, so if the eye colour (for example) on the picture is wrong make sure you change it with an image program. Those who don't include a picture will be expected to provide a reasonably detailed written description of their character so we've all got a solid idea what you look like without you actually needing to write your appearance into the IC.
