Just to add to your question:

Are there several different underworlds, or is there just one, and several of the old gods were ruling over it together?

I guess all of the old gods would be in one place in the underworld, like a giant prison with multiple sections. Each of the gods trapped in concrete or something all Han Solo like.

For now let's just say that the underworld is a BIG place. Like each underworld is it's own continent floating in a void.

@Cuccoruler the underworld also would be capable of its own theme based of the god presiding in it. Say like how I preside over the realm of nightmares within the underworld.
An infinite number of underworlds could all coexist on different existences just beyond the gatekeeper
